Shares transfer

I want to know the procedure for share transfer in a pvt ltd company .

Can you send me a format of share transfer form .

Can a director transfer all his shares to his relatives without boards permission .

If he does transfer all his shares does he cease to be a director of the company automatically or he has to resign and take clearance from board of directors .
