Schedule FA for ESOP Reporting

I have both Stock summary and Cash summary showing in my ESOP holding statement with a broker in US, As i know I need to show the Shares opening , peak and closing in Schedule FA under A3:Details of Foreign Equity and Debt Interest.

So , where do I show the rest cash summary that is there in the statement.

Summary from ESOP Statement:-

Stock-

Opening - $311, Closing  - $355

Closing- $996 Closing - $996.

Also, how to convert the dollar into Rs., as I know we take SBI TT buying rate but dont know of what date as in ESOP no particular date is mentioned only the month is mentioned in most of the cases.
