Share ur mailid... Its simple format... It should be on ur office letter head and signed by ur principal

I just wrote a simple letter addressed to icai vishwas nagar stating that I had my enrolled myself for bcom and the reason for delay. My principal wrote permission granted (below it his name and membership no was mentioned) and I submitted that alongwith form, fine and id card. The purpose is to just know why the delay happened.

I just wrote on plain paper. I took sign of HR not principal nor any other partner. they accepted it.
