Sales against h form

I intend to make a sale against H Form. The buyer is providing me H Form as he wants to export the goods purchased from me. I have the following doubts:

1. Will the buyer provide H form for each purchase or a quarterly H Form?

2. What are the other documents that have to be taken along with the H Form?

3. What is my liability if he refuses or is unable to provide H Form (in case of qarterly H forms)?

4. Should I charge NIL sale tax?

Thanks in advance
