Visit icai online form.....in ur login id
Once you open home page go near "search" bar. Below that you will notice one 'e-Services' hyperlink, click on it. Once you get in that hyperlinked page on the right hand side you will see Students section and the first link is of revalidation. I preferred to straightaway download the form and submitted it to the office of ICAI.
