requirement for ca final form

Hello members, can any body give me the exact list of the set of documents to be attached with ca final NEW COURSE exam form????

please specifically comment whether we need to attach the ARTICLESHIP COMPLETION CERTIFICATE provided by the principal with it?

As the registration letters' last para mentions clearly that completion with extension is necessary in order to be eligible to appear the exam.
