Relating to form 108.

I am a CA Final student, I completed my articleship on 28/4/2020.
But due to the Covid situation in that year & Mental disturbance ,I forgot to fill form 108. I know about the condonation fee of Rs 10000. But after or before that do I have to write a letter to Icai, requesting for condonation?

Dear Sir, With reference of the above mentioned matter it is hereby submitted that I have completed my “Service under Articles” on xx/xx/20xx. The form number 108 is required to be submitted within 30 days thereon. As the same could not be submitted within the stipulated time hence I enclose a demand draft for Rs. 300/- for the applicable fee for acceptance of the same.
