You can re submit a pan application form with a request for change of name (wrongly printed) with sufficient photo id and address proof. Along with a covering letter.

apply for correction in pan card alongwith the proof having your name as mukesh goel in form 49A.

Dont worry Mukesh.

If you have any identification proof in which your name is with correct spell then it will be sufficient.But if you dont have any such prof then you need to go for Affidivit.

Just Fill 49a form (Correction Form),

attach 1) current PAN copy

2) copy of the identification proof that have correct name spell

3) Residence proof

APPLY CHANGE FORM OF PAN WITH CORRECTLY ADDRESS AND ID PROOF WITH A PHOTO DONT RESUBMIT 49A, IT IS FOR NEW APPLICANT.............

U can apply for any correction in PAN card in Form 49a.....

With proper proof of Name to printed in the card and a Photo..

Correction is done through request for correction form. It has boxes on left side to tick places of correction.

yes, you can filed the Form no. 49 a (correction form) with ID proof of name of mukesh goel n residence proof

Dear Friend

Pls tell me the mistake has accured in you PAN of name is from your side or from their side

means as per your records and form filled by you which name written on that form in Block Name printed on PAN Card.

Now fill Form 49A (Form for Correction is PAN and select change in name option)

and also attach your D.L, Passport, Voter id Card etc  min two for correctness (for easy way) and copy of current PAN Card

and if its your mistake means you were previously knowingly with PAN Card name then give adver. in two news paper abt change of name then attach also Affidavit or simply written on paper  and it must be attached by from Gazz. Officer/Doctor etc
