Registration of a public trust Act in Madhya Pradesh

Can someone Please Provide Detail i needed regarding the Registration of a Public Trust under M.P Trust Act.The Information which i needed :

1.Proper Account Head in which the amount Payable to treasury throgh challan for registration.

2.Amount of fees payable,whether fees shall be payable in form of RevenueStamp.
