Without complete details of the place and provision under which they are registering it's difficult to comment. However, as it goes, if your organization falls under eligibility criterien of number of employees, etc. and your facility is within range of an ESIC hospital, you must register. If you don't they can take cognizance and force you to pay ESIC with penalty.
