Regardind wrong fill of ca final exam form

respeected sir.. while filling the exam form of CA final I made a mistake in filling the details of 'last appearance details of final exam' column since I filled the details of may 2014 and Nov 2014 and forgotten to fill the details of may 2015 exams in which I got an exemption in paper 5 I.e advanced mgmt. accounting... please tell me what all I have to do to rectify my mistake...
