Refund from income tax dept

i have filed ITR -1 return for my client for the A.Y 2012-13 with a refund of Rs.570 .

there was a mistake in bank account no. for which i submitted refund re-issue request giving the correct bank account details.

my client received the msg in his phone saying - your refund issue cannot be processed as you have not signed the respondent sheet .

what is this respondent sheet ? wer do i get it and do i have to post it to the cpc bangalore address ?
