Query regarding attachemenst of form iepf-5

I have the shares of Reliance Industries Ltd in the Joint name of my cousin. We have joint demate account also. We have not claimed dividend on that shares from last 15 years therefore the amount of unclaimed dividend and shares are transferred to Investor Education & Protection Fund by the company as per the act. Now we want to claim shares & unclaimed dividend from IEPF Authority.For these process we have to submit Advance Receipt and indemnity bond. My question id whether the Advance Receipt & Indemnity bond will be prepared on joint name as we are joint holders of the shares? whether we can make single joint Advance Receipt and Indemnity Bond for the claim of Shares & Dividend together?
