Go through IDS Scheme Website .U have to Fill IDS Form 1.

No form has been prescribed till now however it will be notified on the income tax website soon.Along with the declaration form proof of tax payment

@ Tax @ 30% of income declared ,Surcharge @ 33% of tax,Penalty @ 10% of income declared

(total 49.9% )

and proof of deposit of 25% of such declared income to interest free deposit scheme will have to be submitted.
