An assesse can disclose his/her undisclosed income in this scheme but how to disclose his undisclosed income in this scheme i.e. whether there is any online form to file this type of Return or in any other way. Please help me to know about the matter.

Go through IDS Scheme Website .U have to Fill IDS Form 1.
No form has been prescribed till now however it will be notified on the income tax website soon.Along with the declaration form proof of tax payment
@ Tax @ 30% of income declared ,Surcharge @ 33% of tax,Penalty @ 10% of income declared
(total 49.9% )
and proof of deposit of 25% of such declared income to interest free deposit scheme will have to be submitted.
