Send a new corrected form 108 immediately to ICAI along with a cover letter clearly explaining that you had made  a partcular mistake in previous form 108 and so you are submitting a new form 108. You should keep very much care in sensitive matters like filing forms to ICAI. This mistake is not tolerable for professionals like us. Dont take it otherwise.
