Please help me with online form

i have filled up my online form for may 2012 IPCC exam. i want to know which all documents are needed to be sent along with the online generated form (PDF print out) and can i submit it in the regional office or do i have to post it? if i have to post it then to which address?

another issue with my online form was, the Father's name which was automatically  generated while filling up the form is mis-spelled, what can be done in this regard? how can this be corrected?

No need to send any document to ICAI.. Just send the PDF printout attested. Thats enogh.

You'll have to courier it to ICAI at the address mentioned...

With ur pdf generated copy . ITT Completion certificate and orientation completion certificate is to be attached. and the form cannot be sent to the regional offices . It has to be sent to the below mentioned address.

The form is to be sent by either speed post or registered post and not courier as mentioned by one of the user...

Bro if u r from kolkata then u can submit the form in the regional office itself...no  need to courier...just take a photocopy of the form after everything i.e sign,photo pasting ....submit the original and get the photocopy acknowledged by them for ur records

no need of any doc... except pdf form

any document will b sent with pdf page?
