Please Clarify my doubt

My articleship finally got over!!! (27 Jan 2011).

Which form is requiring filling? Is it enough for me to file only form 108 or form 119 also?

 

 Only form No. 108 

 My articleship starts from 23/07/2007. And I take transfer on 11/12/2008.

And then re-register from new ca on 17/12/2008. &complete on 27 Jan 2011

So In form 108 what detail I fill. Should I fill the complete articleship detail or the only second employer detail?

 

  you have to submit details for new employer only i.e. 17/12/2008 to 27/1/2011(in the experience column also only submit  details for the period you worked with your new employer) 

 

Whose signature is to be taken on my next examination form (PCC)?

 any CA can sign it and you have to submit articleship completion certificate which institute will send you through post  after they verify your form no 108. 

 

 

 

Pls clarify my doubt.
