Pan card not received not

Hi All, Our client had lost his PAN card . In 2011 December we have applied new PAN Card with same PAN number but difference in Date of birth ,name and father name. We have received one letter from Mumbai in Jan 2012. Later we have called Mumbai TIN office . They have asked gazetted officer verification certificate for changes. We have send the same , again they are asking fathers identify proof as there is a change in fathers name.Actually father of the client has expired longback 15 years back.The client is not having even single records of his father.What is the solution for this problem? Now we are not able to file his income returns. The income tax e-filing site is not accepting old date birth and new date birth also.
