Pan card changes

1) I want to change name in PAN Card and Change in Jurisdiction. Can I do both simultaneously online ?

2) Is it sufficient if I give details in address column online or I have to apply to concerned WARD

3) There is a category online, it says if you desire to update your other address, give required details and submit Proof of other Address, What is this for?
