1) I want to change name in PAN Card and Change in Jurisdiction. Can I do both simultaneously online ?

2) Is it sufficient if I give details in address column online or I have to apply to concerned WARD

3) There is a category online, it says if you desire to update your other address, give required details and submit Proof of other Address, What is this for?

Give PoI for change in the name. Jurisdiction is determined by the address given in the pan application. If you want to change your jurisdiction you have to submit PoA.
