I need to get a PAN card for my wife and hence contacted an agent.

I got her name changed in Bank Account, Voter ID card and Aadhaar card.

Agent says as she has old name in her Birth Certificate (only sername is different) hence she need to go in front of SDM and get an affidavit signed for it.

Is it correct. I have doubt as every lady has same situations if she need to get a PAN card created after her marriage.

Pl suggest. everyone who came across such situation.

it is correct, an affidavit will be reqd to be furnished stating that both names are of the same person.

But is it correct that it need to be attested by a Magistrate ?

As during my application for passport the lawyer attested on his own as my marriage affidavit.

yes, for pan you need to get affidavit from first class magistrate only.

Some of the persons ignore the field 'Have you ever been known by the another name' and apply for PAN based on present name available on available documents. But in that case he has to furnish the docs for DOB in the present name.
