Procedure:
1) download the form attached here with this comment
2) take print out and fill it up your details like registration number, name etc
3) go to bank and get demand draft for Rs. 400 in the name of regional office of your zone, (western / south etc)
4) send this form and demand draft to the address mentioned in the form.
5) you will get letter from ICAI generally within 2 months about revalidation of your registration.
6) There is no online mode. You have send by offline mode only.
7) send this by registered post only.
