Need info on efiling

I am a salaried person and I have donated Rs.10,000 to NGO(Samarthanam) which is less than 10% of my total gross income. NGO have issued a Form58A specifying we can claim 100% tax exemption on the donated amount. Can anyone help me how to get 100% tax exemption if i want to file the IT returns through online filings.  I am using ITR1 form.

My questions are

1) While filing through online portal there is a tab for 80G where it has 4 options of (donation entitled for 100% deduction without qualifying limit,donation entitled for 50% deduction without qualifying limit,donation entitled for 100% deduction subject to qualifying limit,donation entitled for 50% deduction subject to qualifying limit). Among these 4 options which one do i need to choose for claiming 100% tax exemption for that amount.

2) Also do i need to attach the form 58A anywhere ..?
