Can anybody provide me latest manual Form-2 to apply for CA Membership.

After a long search, finally found it and I am attaching the same here.
If you go directly for membership forms in ICAI website, there is only online mode for submitting Form-2. However I could not go through the same as my email-id was not updated. Still, Manual forms are being accepted and on that note, anybody wants to apply in such manner may utilise this form.
