I suggest you to go for a ID & Address Proof, rather than PAN Card  which is only ID Proof.

Please mention details in DIN as per Passport if not any other ID cum Address proof  , which will approve ur

DIN Form.

Hello!

You cant use abbreviation for name and last name of the applicant. U need to write the full name. Try to get the other copies of documents where the particulers are written correctly. He(applicant) can use his fathers name as the last name after providing a affidavit (being notarised)..............just tried to give u some points which may help u to apply for the din.
