Name problem in pan card application( with initials)

My friend has applied for PAN card recently online and got reply "name mismatch" error.

His name is Ravi Chandra S. How does he fill in the online application form to get his name printed as "Ravi Chandra S".

All his ID proof and address proofs have name exactly as mentioned above(Ravi Chandra S)

Problem 1. First name and Last name should not contain initials in PAN application, so how to write his name then.

Problem2. If name is filled as Ravi(1st name) S(middle name) Chandra(last name) then they say name does not match with address/ID proof.

Any suggesstion(s) experience appreciated.
