Annx. G. changed. Previously we have to fill invoicewise details but as per new format we have to filled declartion wise details. This New annex has also corrected state spelling. If you can't upload old Form-704 (ver 1.2.1) try new form download from mahavat site from below link.

As the time is short to prepare Annex. G, Govt. Should extend Due Date for MVAT audit in form 704 for financial year 2009-10 to 31st March 2011.
The due date of Form-704 e-filing is extended to 15.02.2011 as per mahavat site.
But if during this period if new declaration forms were received then there may be agsin delay for update & verifcation of declaration forms. So request to extend upto 31.03.2011.
