Is it the employers discretion to pay an employee part of the salary as Medical allowance or not? Considering the IT excemption available for Medical allowance is there any rule for employers MUST pay part of salary as medical allowance ?

There is no rule that the employer has to pay medical allowance along with the salary. It is upto the discretion of the employer.
