Report the matter to your assessing officer/LVO declaring the 'C' forms of sl.nos lost and the value of invoices covered etc. You will have to submit an indemnity bond in Form 'G' of CST Act on stamp paper and with security wherever necessary.After completing this procedure you can draw fresh forms for your use....mjk
