Karnataka c form generation issue

I am trying to generate C Form for goods purchased from a supplier based in Himachal Pradesh on Karnataka VAT website (vat.kar.nic.in). Steps I followed:

E-CST Forms (Auto Generate) > Request > Enter year as 2016 > Enter TIN of supplier

Company name is auto-populated > Address is not coming, hence I'm manually updating it > State is auto-updated

After this, I'm not able to click on 'Next'. Nothing happens when I click on 'Next'.

If anyone else has faced this issue and found a resolution, please let me know.
