The Journal Entry is

  CASH A/C                                      Dr    25000

  Discount Account                        Dr       2750

                To Revenue Account                                           27750

As the External Doctor Payment is Different it is As per the Mutual Agreement Between your Hospital  & the Doctor. you need to Pay as per agreed payment to the Doctor with out Considering the Patient Fees.

for the record i want to show the other doctor name also and want to include in this entry .so that i can see how much we have to pay at the end of the month

i have made an entery

cash  25000

consultancy exp 700

    to paetint 25000

    to doctor consultant 700

now i want to add discount .

can you please make a entry of discount.please

As you have explain your query above i would suggest you that

Cash A/c (Dr) ..................................25000.00

Consultancy Charges A/c (Dr)........... 700.00

Discount A/c (Dr)............................... 2750.00

                        To Patient A/c (Cr)............... 27750.00

                        To Doctor consultant(Cr).......... 700.00 

bcoz  you have to credit the patient from total amount not only by 25000 so credit the patient by 27750
