All the three available options cannot be exercised since the mob No, Email and the Bank account number were wrong in the previous return. Also the secret question is not known

Then what is the solution to reset password in the absence of DS, secret question, and Ackowledgement/Bank account no.?
Yes its true that otp option is not available for all PANs for some strange reason known only to CPC or efiling people. In the absence of security question answer, otp, bank a/c and e-filing acknowledgment number, password reset is not possible.
