Your CPT marksheet, 12th Marksheet has to be attested.

You have to attach a DD of prescribed amount.

         Yeah and Me too have some question

Because I don't think I have any of these. I have cleared cpt in last june .and then registered for ipcc and after that I haven't done anything regarding ca except for self study. So guys please reply me asap either here or pm me so that I can complete and submit  the form on time.

No marksheets with online form as well
as offline exam form... If you've mentioned 
your ITT training and OT in form.. Attach them!

Old registration no. is of no use if you have enrolled for ipcc okay

Next if you've attempted ipcc earlier and got exemption, mention in the
exempted part (in paper you got exemption)

If not, then no use..

If you have completed your CCT/ITT 100 hours training then mention in 
form, however its not compulsory, same with orientation training of 35 hours..

This'll be my first attemt so I don't have any exemptions and I haven't completed Itt trainig so I don't have the certificate does that mean I'll have to attach my attested CPT marksheet and12th Marksheet?

and one more thing do I have to take the colour print out of  exam form or not because there is coloured logo of ICAI on it

2.ITT CERTIFICATE(if you have got)

note:iit certificate should be submitted to icai before MAY2013 result came out. so complete training as soon as MAY2013 exam over.
