Please confirm wether you want to conduct the audit of ca firm it self or you are asking for normal audit questionare used by CA firms in auditing works ?
Normally a CA firm will be a partnership firm, u can do audit as you are doing a partnership audit. (check up the requirement in sec 224(1B) and complying with code of ethics as per ICAI)

The basic objective of internal audit to find out weekness of management in its operation. keeping in the mind this objective u can create the questionnaire, which will be a exclusive one. Generally the following points may be considered on the time of creating so:-
1 Determine the scope of managent
2 Identify the weekness in internal contral system
3. Don't be descripttive in place of statutoy requirement.
4. Try to think for upgrading the policy of managent
5. Try to work in the benefit of managent
