Incometax assessment order need to submit to passport office

Hello Everyone,

VERY URGENT !!!

Client has applied for passport and gave income tax acknowlegement as address proof, but the passport officer kept the application in hold and asking for " INCOME TAX ASSESSMENT ORDER"

Can you please expalin how to obtain this order?

Can we consider intimation 143(1) as asseement order

OR 

Can we donwload the form and submit the same.
