In spice form, is it compulsory to get "declaration and certification by professional" (ca, cs...)

I am trying to register a new private limited company with 10000 capital and two directors (myself and my wife).
We both have DSC.
In the SPICe form, I have completed all the details.
I have two doubts.
1) Is it compulsory to get "Declaration and certification by professional" (CA, CS, advocate...)
Do I need to get CA/CS DSC in the SPICe form.
2) Do I need to get affidavit by first director on Rs.20 stamp paper and get notary signature on it?

Please suggest.
