there will not be distinction between first, middle & last name. u can fill whole name in First Name Column as in case of firm or company
u have to fill in last name
u r required 2 add ur name in the last name column...... and the date of formation ... this will help u file ur return....

You can write full name as in case of HUF no need of mention first, second and last name...
