sir.

We have four director in a private LTD company our managing director expired on 22/11/2017 and fourth director resigned on 20 /07/2018 . We forgot to intimate MCA in both cases . Now only two directors are available . PL guide me how to upload dir12 Form & need format of documents required for upload

This form is required to filed with registrar within 30 Days from the Date of Appointment/Resignation or the date of such change taking place.

Following Scanned Documents are mandatory and shall be attached along with form:
i. Notice of resignation is to be attached in case of cessation of a Director.
ii. Evidence of cessation is to be attached in case of cessation of a Director.
