How to prepare form 16

Dear All,

I have one note, Please guide me how to prepare form 16 for the F.Y. 2011-12 if the structure  is as below:

paid in a month :

>. Basic salary 15000/-

>  Bonus - 20000/-

>. DA - 5000/-

>. HRA - 5000/-, (Rent paid Rs. 6000/-)

>. TA - 6000/-

>. Medical & Other - 6000/-

> Children Education fees - 500/-

> Children Hostle Allowance - 500/-

Deduction : 

>  PF - 780/-

>  PT - 200/-

Investment :

>  LIC  - 10000

> Infrastructure bond - 20000/-

Housing Loan payment - 10000/-, Intt. on it - 1000/-

Please guide me how to prepare form 16 if structure is as above :
