How to close a Pvt Ltd company?

We have started a company(PVT LTD)  on sept 2009 ROC-Hyderabad. Since its incorporated, no business was done on this.

We have not applied for PAN card.

Now we want to close the company in FASTTRACK EXIT Mode(FTE).

So Please provide the info.

1. What is the procedure and things need to close the company in FTE?
2. Whether I have to pay any penalties to the ROC?
3. How many days need to close the company completely?
4. How much CA would charge for closing the company?
