we are a domestic company having export of service under lut, and having turnover more than 5 cr. in last f.y.
also we have various liability under RCM an import of services.
we have various ITC claims on input services.
can anyone suggest what is the appropriate form to be choose?

It all starts with GSTR-1. It is where the suppliers will report their outward supplies during the reporting month. As such, all registered taxable persons are required to file GSTR-1 by the 10th of the following month. It is the first or the starting point for passing input tax credits to the dealers.
question is which Gstr1 form to be choose considering above points
There is only one GSTR 1 , & as your turnover is more than 1.5 crore so you are liable to file monthly return in GSTR 1
