How to apply income tax practitioner

Sir, yesterday I went to the Office of Principal Commissioner of Income Tax with my ITP Application,They told me that you have to bring details of 3 scrutiny assessment wherein your name appears, Also required Affidavit along with application.

Will somebody have the draft of Affidavit, I want to go through contents thoroughly before filing my affidavit with all relevant requirements.
