If a patient having a medical insurance & his bill amount was Rs.3 Lakhs . As per GST rule, if bill amount is more than 2.5 Lakhs, have to provide the details of the transaction in GSTR-1. In this case whose details has to provide, Patient details or insurance companies details?

In my opinion insurer is paying only on behalf of insured. Patient is the actual receiver of service. So bill should be on patient name and accordingly GSTR1 should be on patient name
If the patient is from outside state (inter state) then and then only you have to furnish the details in B2CL. Otherwise fill B2CS
