Can anyone help me with the sample of filled form 108... and what is the procedure for it??

download form 108 from icai website,filled it and submit it to icai
form 108 should be submitted to icai with in 30 days from the completion of articleship.
Before you file must match detail given in 108 otherwise you receive extention.. So before you file check the other form file by your senior.. So form can be file correctly without any mistake..
