Form SH-4 Details:

Name of the company:
Write the full name of the company whose shares are being transferred.

Consideration amount:
Mention the amount paid for the shares (in INR). If it’s a gift, write ‘Nil’.

Date of execution:
The date when the transfer deed is executed (signed).
Details of the transferor (Seller):

Name(s) of the transferor(s) (seller):
Full name of the person(s) transferring the shares.

Father’s/Husband’s name:
Self-explanatory.

Address of transferor:
Full address of the seller.

Distinctive numbers of shares:
Enter the share certificate numbers (distinctive numbers) of shares being transferred.

Folio number (if any):
Enter the folio number from the share certificate.

Number of shares transferred:
Mention the total number of shares transferred.
Details of the transferee (Buyer):

Name(s) of the transferee(s):
Full name of the person(s) acquiring the shares.

Same as above.

Full address of the buyer.
Signature section:

Transferor(s) must sign here.

Witness details (name, address, signature) — usually two witnesses sign.

Stamp duty: Stamp duty has to be paid on the value of shares transferred as per state laws. This stamp must be affixed on the SH-4 form.
