form no 108 urgent

it is a very big mistake to not have the records(photostate copy). now if you fill the other details it could give bad impression on institute and could training be also extended 

plz tel wat shud he do nw,

send fresh form to institute, lets hope for the best 

he remeber weeks filled bt nt exact one,nd dere could be difference of 2-3 weeks up nd down,cn dey reject it or cn form get frm dere already filled..........plz halp anyone,

HOW COME HE DOES NOT HAVE COPY OF FORM 108.

WHEN FORM 108  IS SUBMITTED AT INSTITUTE, THE INST GIVES BACK COPY OF IT BY PUTTING ACK STAMP OF RCPT ON IT

he just get letter 4 again sending triplicate.....

if he remembers than send form again with details. you can claim that the weeks given were according to memory and signed by principle

it wil create any prblm if deatails mismatch?
