Statement Number:

Purpose-To identy Statement Filed by reporting entity.[In My Example I use 2017/05.I mean year 2017 and 05 means SFT 05-Time Deposit.].

Original Statement ID.

Purpose: It is the statement number which we initially mention in our Form 61A.This should be mentioned when we make a correction in our original statement.In Other cases,we should mentioned it as '0'[ [ZERO}[For Example.Suppose if i want to revise the Form 61A Initially Filed by me,I need to identify STATEMENT WHICH FILED EARLIER.For that purpose, I use statement Number:2017/05,toIDENTIFY MY STATEMENT]

Report Serial Number:

Purpose: It is used to identify PARTICULAR RECORD or REPORT in Form 61A[For Example. We can use 1,2,3,...].Which should be unique in statement.

Original Report Serial Number.

Purpose: TO IDENTIFY PARTICULAR REPORT or RECORD in Form 61A.[For Example We need to change Address of One report which we have filed, whose report serial number is 10.On that occasion,We need to Identify that REPORT or RECORD.In our example it is 10.Hence We need to Mention Oriiginal Report Serial Number as 10 and then make correction.
