Form 16

If I have worked in a company for one month only, do i still need to get form 16 from them. 

I left my company without giving proper notice. I worked in that company for one month only and I was paid salary for that one month.

My concern is what if that company do not send me form 16 ?

How to this income in while submitting filing r eturn ?
