My client (individual) needs to pay insurance premium of $5000 to UK LIfe insurance co. The Bank is asking for form 15 CB/CA. Does he require to give form 15 CA and 15 CB both? He doesn't have digital signature.

Yes.
Form 15CA and 15CB is compulsory w.e.f 01.06.2015 for all remittances.
