Form 108 involving transfer

Hi members.

My articcleship period completed yesterday. I am confused in filling form 108 especially the fourth page. My facts are that, I have commenceed articleship training by 17th August 2012. By 6th Feb 2013 i got transfer to another office. Now my queries are:

1.Do i need to fill only current principal details?

2.In form 108 page 4 (attached herewith) personal details, there is 'Date of commencement of Articleship details'

Which date do i need to enter 17th aug 2012 or 6th feb 2013
