Form 108

dear seniors , can anyone pls upload a copy of their recently filled up form 108 ...there are certain confusions coming up while filling the form...so if someone cud pls upload the filled up form?

also can we download the form from the site and deposit it with icai...or do we have to collect the form from the institute ?
